How well you balance and move may say more about longevity than you think
Regular physical activity plays an important role in healthy aging and is linked to lower risks of cardiovascular disease, functional decline, and premature mortality. However, conventional health assessments may not fully capture an older adult’s overall physical resilience or physiological reserve.
Objective fitness tests can provide valuable insights beyond self-reported activity. Measures of endurance, muscle strength, balance, mobility, and flexibility reflect the combined effects of aging, health, and physical activity.
Researchers analyzed data from 13,423 adults aged 65–90 in Taiwan who completed standardized fitness assessments between 2015 and 2016. Seven tests evaluated cardiorespiratory fitness, strength, flexibility, balance, and agility, with participants followed for all-cause mortality for approximately seven years.
Higher fitness levels were consistently associated with lower mortality risk. The strongest associations were seen for balance, agility, lower-body strength, and cardiorespiratory fitness. Those with the highest overall fitness scores had an adjusted mortality risk approximately 61% lower than those with the lowest scores.
The findings suggest that simple assessments of strength, balance, mobility, and endurance could help clinicians identify older adults with lower physiological reserve and potentially higher health risks.
Because the study was observational, however, it cannot establish that higher fitness directly causes longer survival. Further research is needed to determine whether routine fitness assessments can improve existing clinical risk prediction and support targeted healthy-aging interventions.
