Frailty could be key to early dementia diagnosis
Traditionally, frailty has been viewed as a consequence of ageing and has often been associated with neurodegenerative disorders such as dementia. However, new findings suggest that frailty may play an independent role in the development and timing of dementia rather than simply reflecting declining health.
"Frailty has traditionally been linked to neurodegenerative conditions like dementia, but instead of just being a consequence of ageing or declining health, it appears to play its own role in shaping how dementia develops and is diagnosed," said Dr. Ward.
The study found that older adults living with frailty were diagnosed with dementia approximately two to three years earlier than individuals with lower levels of frailty. Notably, frail individuals who lacked the typical brain abnormalities associated with dementia still developed the condition five to six years earlier than expected.
These findings identify frailty as a potential early indicator of dementia risk and suggest that addressing frailty may help delay the onset of cognitive decline.
"The encouraging message is that frailty is often preventable and treatable," Dr. Ward explained. "Regular physical activity, particularly strength training, combined with proper nutrition and social engagement, can prevent, reduce, and in some cases reverse frailty. Incorporating routine frailty assessments into clinical practice may create opportunities to delay dementia progression and improve patient outcomes."
The research analyzed longitudinal data from more than 1,600 participants enrolled in the Memory and Aging Project conducted by Rush University in the United States over a 24-year period.
The association between frailty and earlier dementia diagnosis remained consistent across all participant groups, although the relationship appeared to be stronger among men.
Study co-author Tom Strating, a medical student at Maastricht University in the Netherlands, noted that genetic risk did not alter the observed association.
"A gene variant known as APOE ε4 increases the risk of Alzheimer's disease, but frailty influenced both carriers and non-carriers in a similar way," Strating said.
Individuals with fewer dementia-related brain abnormalities would normally be expected to develop dementia at a later age. However, this protective advantage was substantially reduced among those with frailty, who developed dementia at roughly the same age as individuals with more extensive brain pathology. Only participants with low levels of frailty appeared to retain the protective effect associated with fewer brain changes.
